How Long Does a Wisdom Tooth Take to Come Through?

In my practice, I often receive patients who come for diffuse pain at the bottom of the mouth, sometimes accompanied by swelling or discomfort while chewing. These signs often raise the same question: is it a question of wisdom tooth that grows ?

What are the signs that a wisdom tooth is pushing?

The most common symptoms to monitor

When the tooth starts to pierce the gum, it can cause local pain, often pulsate, accentuated at chewing. It is not uncommon to observe swelling of the adjacent gum, sometimes accompanied by redness or increased sensitivity to cold or warm.

My patients also describe an discomfort in opening the mouth, a feeling of pressure in the jaw, or even a pain that irradiates towards the ear. These signs are typical of of wisdom and deserve attention.

At what age do wisdom teeth appear?

The period of eruption varies from person to person, but usually occurs between 17 and 25 years of age. In some, teeth will never come out completely. It is then called included teeth, i.e. stuck in the bone or partially visible under the gum.

This "eruption of wisdom teeth" may occur late, sometimes after 30 years, especially when they lack space or when their exit axis is forced. Each situation deserves an accurate assessment.

How can pain be alleviated by a push of wisdom teeth?

What medications to reduce inflammation?

When pain is moderate to intense, it often leads to analgesics such as paracetamol, combined with nonsteroidal anti-inflammatory drugs (such as ibuprofen) if necessary, unless medically indicated. These treatments help calm the local inflammatory process.

In the event of a new infection, antibiotic treatment may be considered, always on medical indication. They are never prescribed first-line without evaluation, as they sometimes mask important signs.

Natural remedies to relieve wisdom teeth

Some simple gestures can help you relieving wisdom teeth waiting for an appointment. The mouth baths of lukewarm salted water, made several times a day, can soothe the irritated area. An ice pocket applied for a few minutes on the cheek reduces swelling and temporizes pain, provided you never apply the ice directly to the skin.

In my practice, I often recommend maintaining good hygiene around the area concerned: delicate brushing, use of a dental jet so accessible, and avoidance of hard or irritating foods.

I often recommend salty mouth baths and a gentle diet to limit the pains of erupting wisdom teeth.

When is it necessary to consult a dentist for a wisdom tooth that grows?

Recognizing emergency signs

Persistent pain that worsens, fever, difficulty opening the mouth (trismus), fetid breath or purulent discharge may indicate infection. We're talking about pericoronarite, inflammation of the gum covering a tooth partially exit.

In this case, consultation becomes urgent. The risk of abscess or spread of infection in deep tissues is not to be overlooked. Rapid management often avoids more invasive emergency response.

The role of the dental surgeon in the diagnosis

During the consultation, the clinical examination is supplemented by a panoramic X-ray or a 3D scanner to assess precisely the position of the wisdom teeth, their orientation, and their relationship to the neighbouring structures (molar roots, inferior alveolar nerve...).

It is from this analysis that we decide together, in full transparency, the course to be followed. I always prefer a cautious approach, adapted to the situation and your oral history.

Must we extract a wisdom tooth?

When is extraction recommended?

There is no need to systematically extract a wisdom tooth. Some grow straight, without creating pain or imbalance. On the other hand, extraction is proposed when the tooth lacks space, is poorly positioned, causes repeated infections or exerts harmful pressure on adjacent teeth.

The teeth included, which fail to come out completely, sometimes cause cysts or progressive dental movements. In such cases, preventive extraction may be considered.

How is the wisdom tooth removal going?

The procedure usually takes place under local anesthesia. After an incision in the gum, the tooth is released from the bone if necessary, then extracted delicately. A suture is then placed to promote healing. The operation lasts between 20 and 45 minutes, depending on the case.

In some complex situations (near teeth, atypical root shape), I recommend a more thorough surgical evaluation, sometimes in hospital settings.

How long to recover after a tooth extraction of wisdom?

Post-operative tips for healing

The recovery varies from patient to patient, but usually take 5 to 7 days for comfort. During this time, I recommend avoiding hard food, hot drinks, and intense physical effort.

Hygienically, it is best to gently rinse your mouth with an antiseptic (alcohol-free) mouthwash from the next day. Brushing should be continued, but bypassing the area within the first few days to avoid any disturbance of healing.

What to do in case of persistent pain?

A pain that persists beyond a few days or intensifyes should evoke a possible complication, such as alveolite (inflammation of bone alveole). In this case, I recommend not to wait and consult again.

Signs of infection (fever, late swelling, discharge) or persistent unpleasant taste in the mouth should also alert you. A quick response allows in most cases to properly restore the situation.

What impact do wisdom teeth have on oral health?

Displacement of neighbouring teeth and malocclusions

A poorly positioned wisdom tooth can gradually induce movement of other teeth, especially when there is not enough room in the jaw. This movement may affect alignment and, in some patients, compromise the results of previous orthodontic treatment.

During my consultations, I always evaluate this aspect from X-ray photographs and dental occlusion, especially among adolescents and young adults still in periods of orthodontic stabilization.

Infections, cysts and other complications

An included or semi-impacted tooth can create an area that is difficult to clean, conducive to the accumulation of bacteria. This can lead to recurrent infections, but also to the formation of cysts around the root. These lesions, although often benign, can damage the nearby teeth or bones if neglected.

Regular monitoring of wisdom teeth avoids such complications. I usually recommend radiological control every 2-3 years, even in the absence of symptoms.

In case of doubt on a wisdom tooth that grows, it is always more reassuring to consult. This helps to calm pain and prevent potential complications. Each mouth is unique, each tooth, its own path. What I seek above all is a shared decision built in dialogue and trust.
